Medicare participation agreement

The Medicare participation agreement is a contract by which physicians treating Medicare beneficiaries (generally patients over 65 years old) receive payment directly from Medicare carriers, acting on behalf of the federal government. See Form CMS-460.
Physicians cannot opt out of Medicare if they practice under a participating physician agreement. First they must become "non-participating" physicians before they may opt out.
