Vaccine Adverse Event Reporting System (VAERS) is a database managed by the Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration (FDA). VAERS provides a mechanism for the collection and analysis of adverse events associated with vaccines currently licensed in the United States.
A user-friendly compilation of VAERS data is available here.
Reports to VAERS can be made by either the vaccine manufacturer, a vaccine recipient or their parent/guardian, or a health care provider. For more information on VAERS call (800) 822-7967.[1]

According to VAERS, of the U.S. deaths reported between December 14, 2020 and September 24, 2021, 11% occurred within 24 hours of vaccination, 16% occurred within 48 hours of vaccination and 29% occurred in people who experienced an onset of symptoms within 48 hours of being vaccinated.[3] The mortality rate from receiving the covid vaccine is 2.12%, and the risk of permanent disability is 2.97%. 11.33% needed Emergency Room visits and 9.41% required hospitalization.[4]
