Part E has five blocks: reason for departure (1, this is voluntary and an employee need not give any reason for leaving, but it may be considered should the employee wish to return to civil service), effective date (2, the effective time is midnight unless specified otherwise), signature and date (3 and 4), and a forwarding address (5).
