Fail Deadly

Fail Deadly is a term used to describe a weapon safety system that, in the absence of an abort code input by a human, will launch the weapon. One of the most famous examples of this was the Soviet Systema "Perimetr"(a.k.a. Dead Hand) nuclear weapons system.
