EBSA Form 700 is a form that the federal government has required non-profit organizations to complete and submit, beginning January 1, 2014, in order to claim an exemption from the contraceptive mandate under ObamaCare. Churches themselves are exempt without having to submit this form.
Several non-profit organizations with religious beliefs immediately challenged the requirement that they complete this form.
