Fit as a fiddle is an idiom that indicates a very good level of health and physical fitness. It might typically be used to describe an elderly person who goes mountain climbing, plays tennis, and so on.
The term may come from the notion that violins (and other instruments) play better when they are well-maintained, with taut strings, etc.
