The shooter walked from his van carrying an AR-15 and wearing camouflage pants in plain sight of rallygoers. He came to a building where a ladder had been placed before time. He climbed the ladder in full sight of rally goers carrying his AR-15. Rallygoers notified police of his presence.

The line of sight (LOS) distance from the building roof top was approximately 130-yards to the podium where President Trump was speaking. Why roof top was not secured and protected raises lots of questions as to why, and whether there was proper oversight within the Secret Service.
